ประกาศความเป็นส่วนตัวด้านข้อมูลผู้ร่วมงาน / Privacy Notice Regarding Participant Data

หน่วยบริการวิชาการ คณะกายภาพบำบัด มหาวิทยาลัยมหิดล (ซึ่งต่อไปนี้เรียกว่า “เรา”) ให้ความสำคัญเป็นอย่างยิ่งกับการคุ้มครองข้อมูลส่วนบุคคลของท่าน และเพื่อให้ท่านมั่นใจได้ว่าเราจะให้ความคุ้มครองและปฏิบัติต่อข้อมูลส่วนบุคคลของท่านโดยสอดคล้องกับกฎหมายว่าด้วยการคุ้มครองข้อมูลส่วนบุคคล เราจึงได้กำหนดประกาศความเป็นส่วนตัวเกี่ยวกับข้อมูลส่วนบุคคลฉบับนี้ขึ้น เพื่อแจ้งให้ท่านในฐานะผู้เข้าร่วมการอบรมกับเรา ทราบรายละเอียดการดำเนินการกับข้อมูลส่วนบุคคลไม่ว่าจะเป็นการเก็บรวบรวม ใช้ และเปิดเผย (รวมเรียกว่า “การประมวลผล”) ซึ่งอาจเกิดขึ้น ตลอดจนแจ้งให้ท่านทราบถึงสิทธิในข้อมูลส่วนบุคคลของท่าน และช่องทางการติดต่อเรา ดังต่อไปนี้

โดยทั่วไปแล้ว เราจะเก็บรวบรวมข้อมูลส่วนบุคคลของท่านโดยการขอหรือสอบถามข้อมูลเหล่านั้นจากท่านเองโดยตรงจากระบบและช่องทางที่เรากำหนด เช่น แบบฟอร์มการรับสมัครการอบรมแบบออนไลน์หรือกระดาษ หนังสือตอบรับการแจ้งรายชื่อผู้เข้าร่วมจากส่วนงาน/หน่วยงานต้นสังกัดของท่าน เป็นต้น ทั้งนี้ เราอาจดำเนินการเก็บรวบรวมข้อมูลส่วนบุคคลของท่านซึ่งเป็นข้อมูลส่วนบุคคลทั่วไป อาทิ

• ข้อมูลที่ใช้ระบุตัวตน (Identity Data) เช่น คำนำหน้าชื่อ ชื่อ – สกุล ประเภทผู้เข้าร่วม ภาพถ่าย (เดี่ยว) เลขที่ใบประกอบวิชาชีพ เลขประจำตัวบัตรประชาชน เป็นต้น
• ข้อมูลติดต่อ (Contact Data) เช่น เบอร์โทรศัพท์มือถือ เบอร์โทรศัพท์ที่ทำงาน อีเมล เป็นต้น
• ข้อมูลการเข้าร่วมกิจกรรมต่าง ๆ เช่น ภาพนิ่ง ภาพเคลื่อนไหวบรรยากาศการจัดอบรม วิดีโอบันทึกการอบรม ลายมือชื่อเพื่อลงชื่อการเข้าร่วมกิจกรรม เป็นต้น
• ข้อมูลการทำงาน เช่น ตำแหน่งงาน ตำแหน่งบริหาร หน่วยงานที่สังกัด ที่อยู่ของหน่วยงาน เป็นต้น
• ข้อมูลเกี่ยวกับความคิดเห็น ความคาดหวัง ความพึงพอใจในการเข้าร่วมการอบรม
• ข้อมูลส่วนบุคคลอื่น ๆ เพื่อการอำนวยความสะดวกตามจำเป็น เช่น ประเภทอาหาร (กรณีมีการจัดเลี้ยงเบรกและ/หรืออาหารกลางวัน) ยี่ห้อและเลขทะเบียนรถ (กรณีมีการสำรองที่จอดรถให้กับผู้เข้าร่วม) เป็นต้น
• ข้อมูลการคัดกรองตามมาตรการป้องกันโรคระบาด (กรณีมีการจัดอบรมแบบ Onsite) เช่น ผลการตรวจ Antigen Test Kit (ATK)
• ข้อมูลที่เกี่ยวข้องกับการเงิน เช่น เลขที่บัญชีและหน้าสมุดบัญชี เพื่อทำการคืนเงินค่าลงทะเบียน

ในบางกรณีเรามีความจำเป็นต้องใช้สำเนาบัตรประจำตัวประชาชนเพื่อการพิสูจน์และยืนยันตัวตนของท่าน ซึ่งมีข้อมูลส่วนบุคคลที่มีความละเอียดอ่อน เช่น ศาสนา และหมู่โลหิต รวมอยู่ด้วยนั้น

โดยทั่วไปแล้ว เราไม่มีความประสงค์จะเก็บรวบรวมและใช้ข้อมูลศาสนาและหมู่โลหิตที่ปรากฏอยู่ในสำเนาบัตรประจำตัวประชาชนของท่านเพื่อวัตถุประสงค์ใดโดยเฉพาะ หากท่านได้มอบสำเนาบัตรประจำตัวประชาชนให้แก่เรา ขอให้ท่านโปรดปกปิดข้อมูลดังกล่าวพร้อมลงนามกำกับ หรือท่านอาจมอบหมายให้เราปกปิดข้อมูลดังกล่าวแทนโดยมีการรับรู้ร่วมกันทั้งสองฝ่าย

The Academic Service Unit, Faculty of Physical Therapy, Mahidol University (hereinafter referred to as "we") places the utmost importance on the protection of your data. To ensure your confidence that we will protect and handle your personal information in compliance with the Personal Data Protection Act, we have issued this privacy notice to inform you, as a training participant, about how your data may be collected, used, and disclosed (collectively referred to as "processing"). This notice also outlines your rights regarding your data and provides contact channels for inquiries, as detailed below.

In general, we collect your data directly from you through systems and channels that we have established, such as online or paper-based training registration forms, confirmation letters from your department or organization, etc. We may collect general personal data, including but not limited to:

• Identification DataSuch as title, first and last name, type of training participant, photograph (portrait), professional license number, national ID card number, etc.
• Contact DataMobile phone number, work phone number, email address, etc.
• Event Participation DataThis includes photographs, videos of the training environment, recorded training videos, and signatures for attendance.
• Employment DataJob title, management position, affiliated department, department address, etc.
• Feedback and Satisfaction DataSuch as opinions, expectations, and satisfaction levels regarding the training.
• Additional Personal Data for ConvenienceSuch as food preferences (in the case of catering during breaks or lunch), vehicle brand and license plate number (if parking spaces are provided for participants), etc.
• Health Screening DataSuch as results from an Antigen Test Kit (ATK) (in the case of onsite training).
• Financial DataSuch as bank account number and account book details for refund purposes.

In certain cases, we may need to request a copy of your national ID card to verify your identity. This may include sensitive personal data, such as religion and blood type, which we generally do not intend to collect or use for any specific purpose. If you provide us with a copy of your national ID card, please ensure that you redact this sensitive information and sign to confirm, or you may authorize us to redact this information on your behalf, with mutual acknowledgment by both parties.
